As part of Affordable Care Act, Medicare has developed a program whereby those that are on Medicare and have significant comorbidities can have better access to their primary care physician. It is a program only meant for those with significant illness. By signing on for chronic care management, patients will receive periodic calls to address issues such as supplies, medication refills, timely referrals and lab studies, as well as imaging. Ultimately, the goal is to reduce hospitalization by establishing a more direct conduit to care between a primary care physician and their Medicare patient. At Partida Corona Medical Center, these issues are often dealt with through our office staff during visits, but with chronic care management, we are also more able to review patient’s individual needs between visits so that issues that may have otherwise led to hospitalization get addressed well before such a step is even needed.
